* Please email a current picture of your operation in action to clong@carolinacrossroads.com. Without a photo, your application will not be considered. * Each vendor must provide Certificate of Insurance. Each vendor must provide Certificate of Insurance showing a Comprehensive General Liability policy listing Carolina Crossroads and Roanoke Rapids Entertainment One, LLC as “Additional Insured” and specifying the amount of $1,000,000.00 for liability insurance. A copy of the vendor’s North Carolina seller’s permit is also required. * Vendors that require water must supply 150' of food grade water hose. By submitting this application, Carolina Crossroads does not guarantee a vending spot or contract for the event. Carolina Crossroads reserves the right to restrict the sale of any items. I understand that this form is an application for SPACE ONLY and is neither a commitment by the applicant nor an offer by Carolina Crossroads to rent space. I understand that Carolina Crossroads reserves the right to determine the final location of any concession.
